BOARD OF HEALTH MINUTES
Meeting Held at Town Hall on September 27, 2011 at 6:30 PM

Minutes reviewed for 9/13/11:
- C. Rogers moved to approve the minutes of 9/13/11, C. Slade 2nd. All in favor 2/0.
Mail Reviewed:
Action/Discussion:
- 23 Annie Moore Rd., Hamilton – form 9A and 9B re: system upgrade.
- Permit was approved at 8/9/11 mtg., but the forms 9A & 9B were not signed. Board signed.
- DEP updated Board member contacts and PWS update.
- M. Carlisle to update contact information.
- Board reviewed the DEP PWS list to make sure none were missing.
- Response to Open Meeting Law complaint.
- C. Slade read the letter he drafted to the Board.
- Board approved. M. Carlisle to type up.
- B. Brookings asked if the Board wanted him to attend the meeting on Thursday, 9/29/11 regarding the Smith property. Board said that if he has time, he could go.
- B. Brookings asked the Board if they wanted an updated plan for lot 48A Century Mill Estates; 37 Mill Pond Road since the lots have changed. Board stated that they do want an updated plan. M. Carlisle to send letter.
Building Permits Reviewed:
- 105 Vaughn Hill Rd., Daman – remodel 3 baths. Bill signed 9-16-11.
- 62 S. Bolton Rd., Slade – 400 square foot deck. Bill signed 9-27-11.
Septic Permits Reviewed:
- 32 Harvard Rd., Barss – extension request. Board approved. COC by 9/23/2012. M. Carlisle to send letter.
- Lot 2C Bare Hill Rd., Dixon – extension request. Board approved. COC by 10/23/2012. M. Carlisle to sent letter.
- Lots 20 & 21, 22 & 23 Century Mill Estates – new shared system permit. Move to next agenda.
- 35 Fox Run Rd., Plante – transfer of permit to Mark O’Hagen. Board approved.
- Houghton Farms re-dated shared system permits.
- Board decided they want to wait until the Nitrogen Aggregate system is approved by DEP before they sign the septic permits.
Inspections reviewed:
- 20 Teele Rd., Keating – Title 5. Board approved.
- 359 Berlin Rd., Pinsky – Title 5 annual inspection for the cultec bed. Board approved.
- 80 Laurel Rd., Preman – Title 5. Discrepancy in number of trenches. M. Carlisle to send letter.
- 7 Nourse Rd., Oldham – Title 5 six month reinspection. Board approved.
7:00 – 7:15pm Appointment – Chuck Black, Kendall Homes, sale of lot 17, 94 Drumlin Hill Road regarding the Long Sewer Line Inspection by Title 5 Inspector:
- Chuck Black of Kendall Homes appeared before the Board to discuss the deed restriction required on lot 17, 94 Drumlin Hill Road. This restriction was required as a result of a variance given by this Board on March 22, 2011 for a long sewer line. This deed restriction requires that an annual inspection of the long sewer line be done by a Title 5 Inspector.
- Mr. Black is requesting that this inspection by done by the homeowner and not a Title 5 Inspector.
- C. Rogers stated that because the area is visible, typically, the inspection would be done by homeowner.
- Mr. Black presented deed restriction language drafted by his attorney. Board reviewed and approved.
- L. Andrews motion to accept the amended deed restriction for lot 17, 94 Drumlin Hill Road, C. Rogers 2nd. All in favor 3/0.
- M. Carlisle to send letter.
L. Andrews moved to adjourn the Board of Health meeting at 7:24pm, C. Rogers 2nd. All in favor 3/0.
